If you’re a commercial driver with diabetes, you might be wondering: Can I still get my DOT medical card? The good news is—yes, you can drive with diabetes, but there are important FMCSA guidelines you need to follow.

๐ FMCSA and Diabetes: What Drivers Need to Know
The Federal Motor Carrier Safety Administration (FMCSA) oversees the medical fitness of commercial drivers. In the past, drivers with insulin-treated diabetes needed a federal waiver. But as of November 2018, the rules changed.
Today, CDL drivers using insulin can still qualify for a medical card, as long as:
✅ They work with their treating clinician (typically a primary care doctor or endocrinologist)
✅ The clinician completes an Insulin-Treated Diabetes Mellitus Assessment Form (MCSA-5870)
✅ The form is brought to the DOT physical exam

This form confirms that the driver’s blood sugar is well-managed and that there have been no severe hypoglycemic episodes (like loss of consciousness or needing outside help) in the last 12 months.
The form must be:

If you’re on insulin, your medical certificate is typically valid for up to 1 year. Non-insulin-dependent drivers may still qualify for a 2-year certificate, depending on overall health and blood sugar control.
